The Steven A. Cohen Military Family Clinic at the University of Pennsylvania is seeking an Outpatient Case Manager. S/he will work as part of a larger behavioral health care team within the Cohen Clinic at Penn to serve veterans and military family members. Work will take place in our outpatient clinic setting. Case Manager will assist clients and team members in identify appropriate external resources for clients. The case Manager will complete Case Management Assessments to identify goals and needs with clients and assisting in connecting clients with resources as needed. Will work closely with other Care Coordination team members to maintain up to date referral resources on our shared drive. Will track referrals and maintain timely and accurate documentation in the electronic health record. Case Manager will maintain strong working relationship with external partners, participating in some direct community outreach to support these working relationships. Travel will be limited to occasional local outreach/partnership events.
Coordinator A/ Outpatient Case Manager (Grade 25)
– Provider direct Case Management Services under the supervision of the clinic director
– Maintain electronic health records accurately and in a timely manner
– Participate in regular team and consultation meetings to assist in care coordination and internal process improvement
– Maintain relationships with external partners and referrals sources
– Maintain internal referral directory
Coordinator B/ LSW Case Manager (Grade 26)
– In addition to the duties above:
— Serve as the Case Management lead within the clinic
— Assist with Biopsychosocial Assessments, as needed
*Please note: This position will not provide psychotherapy services.
Experience in the military or working with military service members strongly preferred. Military Veterans and Military Family Members are strongly encouraged to apply.
Must pass Pennsylvania s Child Protective Services Act (also known as Act 73) background checks, within 30-90 days of employment. Will require some local travel. Access to a vehicle preferred; valid driver’s license required.
Outpatient Case Manager (grade 25)
A Bachelor’s degree in Social Work, Psychology or a related field and 1 to 3 years of experience, or an equivalent combination of education and experience, are required.
LSW Case Manager (grade 26):
The minimum of a Master’s degree in Social Work and 2 years of experience. License in Social Work (LSW or higher) is preferred.
Experience in Case Management is required.
