The Case Manager is part of a multidisciplinary clinical team who serves as the bridge between clinical services and essential community resources. The Case Manager facilitates direct referrals for housing, employment, healthcare, legal, educational, peer-support and all other essential community-based needs for clinic clients. The Case Manager manages referral partnerships by continuous engagement post-referral, ensuring the provision of quality care. The Case Manager assists the clinical team in understanding resources and needs within the clinic community. The Case Manager is estimated to support ~250 patients/year or 70 patients at a time (based on expected utilization levels).

The Assistant Director will report to the Clinic Director, and will interface with the Cohen Veterans Network (CVN) central office. The Assistant Director will be a full-time employee of the clinic, with responsibilities including administrative and patient-facing clinical work. The Lead Clinician serves at the Evidence Based Practice Champion and primary clinical leader on an interdisciplinary team, focused on providing time-limited, evidence-based practices in an outpatient clinic that serves individual adults, couples families and children. The Lead Clinician will provide evidence-based treatment to veterans, particularly those who served in the Iraq and Afghanistan wars, and their families. In addition to direct clinical care, the Lead Clinician is responsible for supporting the Clinic Director through the provision of supervision, case consultation, and mentoring in CBT and other time-limited, manualized interventions. The Lead Clinician will be a full-time employee of the clinic, and reports directly to the Clinic Director.

The Outreach Coordinator will be responsible for referral of clients to the Cohen Clinic through outreach to community partners including veteran-service organizations, military service organizations, the U.S. Department of Veterans Affairs, law enforcement, educational institutions, the faith-based community and other local partners. The Outreach Coordinator will also participate in peer-to-peer communication, presentations, and other outreach engagements as needed.  Recruitment will focus on reaching veterans who served in Iraq and Afghanistan wars and their family members.

Provides support to the Cohen Clinic as the first contact for services. The Intake Coordinator will conduct phone and face to face screenings and intakes on-site and off-site for veterans and their families. The Intake Coordinator will work with a multi-disciplinary team for case assignments, consultations and care coordination.

As Clinic Director, you are responsible for the following: Oversee all clinical operations (e.g. clinical care, staffing, training, budget, measurement and evaluation). Ensure clinical staff are aware of, and following, evidence-based practice guidelines throughout treatment. Provide overall clinical supervision for the Cohen Clinic staff, including weekly clinical supervision for therapists, crisis management support as needed, and management of a clinical training program in evidence-based treatment modalities. Manage intake process for new patients, including supporting intake workers as needed and leading weekly disposition meetings for the clinical team to assign and present cases. Establish and maintain effective working relationships with those contacted in the course of work including diverse interest groups, advocacy groups and other governmental organizations. Ensure the clinic and its staff actively communicate with the CVN Central Office, participate in CVN meetings/events/activities, and contribute to the overall network. Through leadership, develop and foster a culture of collaboration, peer consultation, and commitment to ongoing education and professional improvement.

The Clinician will provide evidence-based treatment to veterans, particularly those who served in the Iraq and Afghanistan wars, and their families. The Clinician will have training Cognitive Behavioral Therapy (CBT) and should be skilled at offering treatment in a measurement-based, time-limited treatment environment. Clinician must have background and training in utilizing evidence-based interventions in the assessment and treatment of mood disorders, Post Traumatic Stress Disorder and adjustment disorders. Qualified candidates will be proficient in providing individual, couples, and family therapy, with an emphasis on delivering evidence-based care. Certification or documented proficiency in one or more in one or more of the following EBPs: PE, CBT and CPT is required at time of hire or within a year of employment.

Responsible for managing and coordinating all clinical activities that advance the operations for the Cohen Veterans Clinic (CVN) of Cape Fear Valley Health System. Both the Corporate Director of Behavioral Health Care & Sleep Medicine (or designee) and the Clinic Director interface with the CVN Central Office, and are the primary conduits of the CVN mission, policies, and communications to the clinic.
